We need some more information from you

So that we can assess your application and medical history, we need more information. To obtain this, we will either call you, ask a nurse to visit you or ask your doctor for a report.

If we need to talk to your doctor, we'll need you to sign a consent form, this authorises us to apply for medical information and for your doctor to provide it.

You can print the Consent Form PDF now rather than waiting for it to arrive by post. When completing the form you'll need your application number which can be found in the confirmation email we've just sent you. Ways to return the form:

  • Scan a copy and email it to
  • Fax a copy to 029 2049 2938
  • Post it to: FREEPOST, PIU, Legal & General

Next steps

Letting you know our decision Once we have all the information we need we'll assess your application and write to confirm our decision. We may need to charge a higher premium based on the information in your application and any further information we've collected.

However, if you wish to keep the quoted premium for a smaller amount of cover, just let us know. If we're unable to offer you cover, we'll send you a letter to confirm our decision.
Checking your details If we offer you cover, we'll send you:
  • A copy of the answers you gave in your application.
  • A declaration form.

  • Check that the answers are correct and use the declaration form to tell us of any changes. If you do inform us of any changes, it may be necessary for us to obtain further information.
  • Sign and return the declaration form, even if you don't need to tell us about any changes.
Your policy starts
  • If we offer you cover, your policy will start either on the date you originally requested or a revised start date, if your original date has passed.
  • If we offer you cover, we'll write to inform you when premiums will be taken from your nominated bank account. Please be aware that the first premium may not be taken on the date you requested. Further premiums will then be taken on your preferred date, or the next working day.
  • We'll send you your Policy Schedule and Policy Terms and Conditions, which may arrive before the policy start date.
